The following form must be filled in by the EVS volunteers hosted in a Romanian project for more than 6 months.

MidTerm Evaluation takes place at the middle of the EVS service and it lasts, on average, for three days.

For transparency reasons and the correct use of EU funding your participation is conditioned by the following conditions (considered to be accepted you check the YES option).

I agree that, if I will be selected for participation, I fully understand and I assume the following:
- I understand the obligation to be active and participate during the entire program of the event
- I am fully responsible for any damage that I may cause during lodging and I know that in case of incident(s)/damage(s) caused, the hosting/coordinating organization has the full right to request me for financial reimbursement
- I am aware of the fact that the receiving organization is also contractually responsible toward the hosting National Agency for any consequences generated by my behavior during the participation in the MidTerm Evaluation!
- I agree to be photographed of filmed during the event and that these materials to be published with the goal of promoting the event and results dissemination, without affecting my personal or institutional image
- I agree with the personal data processing in order to ensure the organization and communication of the event
- I agree to be contacted via e-mail or phone by the event organizers and potential auditors
- I agree that my name will be published on the event page at the selection stage.
